Please download and complete the following forms NEW PATIENT PACKET before arriving at your first appointment. You may hand deliver, mail, or fax your completed NEW PATIENT PACKET to our office so that it is received at least one week before your appointment date. (If you choose to fax the NEW PATIENT PACKET, please call to verify whether we received it).

This will allow us adequate time to get your information entered into our computer system and will expedite the time you are in our office for your first appointment. If your completed NEW PATIENT PACKET has not been delivered to our office before the date and time of your appointment, you may be rescheduled.
Your packet must be completed, including the insurance information section. Please bring with you all the medications you are currently taking.

Marquez Anesthesia, PLLC is currently in the process of negotiating contracts with Humana, Aetna, Cigna, BCBS of TX, and Amerigroup. The list of insurance providers we accept changes frequently. Our office staff will work with you to explain your insurance benefits to you. We will work with you to make sure that if you are out-of-network, we set up a plan that makes it affordable for you to see us as we now offer payment options. If you do not see your insurer listed below, please contact our office directly at (214) 449-0110 to ensure that your insurance will be accepted. Our policy is that all co-pays and deductibles are due at the time of check-in, as well as private pay. Unpaid deductibles and co-insurance percentages will also be due upon checking in.
As a courtesy to our patients, we will file claims for all insurance types. We accept third-party insurance and Workers Comp. We do accept cash, cashier checks, money orders, and credit card payments.
